Recently, in the port of Zeebrugge, Belgium, the Norwegian Prima dislodged from the Pier when the mooring snapped. This caused the gangway, the platform connecting the vessel to the land, to collapse into the water below. Fortunately, nobody was on the gangway when this took place, however, situations like these show how fragile and unpredictable cruise ship safety can be. According to Norwegian Cruise Line it was erratic high winds which caused the mooring to break; although I personally find this excuse insufficient given that mooring is supposed to withstand natural events.
